Why Most New Health Programmes Are Designed in the Wrong Order

Most new healthcare education programmes begin with facilities and a course list, and define graduate competencies last. Reversing that sequence changes everything — including the odds of accreditation.

Most new health sciences colleges begin in the wrong place: with a site, a capital commitment, buildings, and a list of programmes adapted from somewhere else. Only near the end does someone ask what graduates must actually be able to do.

That sequence ignores dependency. Facilities determine learning experiences. Learning experiences determine which competencies can be developed. Competencies determine whether graduates meet health-system needs. When purpose is defined last, it becomes the residue of decisions about budget, land, prestige, or imitation.

Reverse Educational Design starts outside the institution. It examines health-system priorities, population needs, and employer expectations, then translates those needs into the professional capabilities graduates must demonstrate. Only then do we define the clinical immersion, simulation, mentorship, research engagement, and interprofessional learning required to build those capabilities.

Now, and only now, do we design the institution: curriculum architecture, faculty profile, facilities, partnerships, governance, assessment systems, and capital requirements. Every specification inherits its justification from the evidence that came before it.

The result is clearer accreditation logic, stronger capital decisions, and a curriculum faculty can teach with a visible purpose. More importantly, the approach is a continuous loop. Employers, graduates, patients, and the health system continue to refine the competencies and design.

Designing backwards is not a stylistic preference. It is the difference between an institution that produces graduates the health system needs and one that merely delivers the programmes it happened to build.
